nH-Engage helps hospitals, payors, ACOs, and post-acute care providers identify patients who are considering going to the emergency room before they do so, uncover patients who are unknowingly failing at home, and alert clinicians or navigators of at-risk patients so that they can engage the patient before a costly negative event occurs such as unnecessary emergency department visit or readmission.
nH-Engage can be deployed for patients receiving post-acute care services or patients discharged to self-care only to monitor and track recently discharged patient’s progress and actions via a patient mobile application or automated phone calls.
